    Position Information

    The Department of Health Services is seeking an experienced Supervising Public Health Nurse to lead public health nursing services supporting Sonoma County’s Adult and Aging Division!

    Starting salary up to $85.19/hour ($177,801/year) and a competitive total compensation package!*

    About the Position
    Under the direction of the Director of Public Health Nursing and in close partnership with the Human Services Department's Adult and Aging Division, the Supervising Public Health Nurse (PHN) plays a pivotal role in planning, organizing, and overseeing public health nursing programs that serve vulnerable and older adult populations. This position is responsible for supervising a team of Public Health Nurses and serves as a liaison between Public Health and Human Services, as well as with community-based organizations. The Supervising PHN helps assess community needs and develops collaborative strategies to improve population health outcomes. Additional duties may include:
    • Participating in hiring, onboarding, educating, and evaluating staff
    • Supporting budget management, grant compliance, and program evaluation
    • Facilitating the use of evidence-based practices
    • Helping to identify service gaps and system improvements
    • Representing programs at internal and external meetings, and making presentations as needed
    • Supporting program compliance by ensuring the team follows relevant federal, state, and local regulations

    As the ideal candidate for this position, you will be a proactive and collaborative leader with a strong foundation in public health nursing and experience working with older adult populations. In addition, you will possess many of the following:
    • A background in public or community health, with an emphasis on population health
    • Familiarity with electronic medical records and relevant regulations and policies
    • Competency in Microsoft Office Suite; intermediate or advanced skills are desired
    • Effective written and verbal communication skills, including public speaking and presentations
    • The ability to consult with subject matter experts and navigate complex or unique cases
    • Experience working with diverse communities and organizations with the ability to create and maintain solid partnerships with them
    • A commitment to continuous learning, equity, and public service, and to bringing this attitude forward in your leadership
    • Two years of recent supervisory experience is desired but not required

    This recruitment is being conducted to fill a Supervising Public Health Nurse position in the Department of Health Services.     Minimum Qualifications

    Possession of a California State Public Health Nursing Certificate.

    AND

    Three years of responsible public health nursing experience obtained after receipt of a California Public Health Nursing Certificate.

    Driver’s License: Possession of a valid driver's license at the appropriate level including necessary special endorsements, as required by the State of California to perform the essential job functions of the position.

    The position advertised on this announcement requires possession of a valid California Driver’s License.

    Working Conditions
    Duties may require exposure to hostile, emotionally disturbed or mentally ill persons and/or confrontational interpersonal situations; stand for long periods of time; stoop, lift clients, and lift heavy articles in accordance with accepted safety standards; perform physical tasks as required; may work in dangerous, high crime areas of the community; and/or standby, callback and work during non-routine hours; intermittently sit at a desk for long periods of time while reviewing patient charts and histories and preparing patient charts and reports; intermittently walk, stand, bend, climb and reach while making field visits, conducting clinics or making educational presentations or demonstrations; twist to reach equipment surrounding desk or treatment areas; perform simple grasping and fine manipulation; use telephone and write or use a keyboard to communicate through written means; see and hear with sufficient acuity to examine and assess patient conditions; lift moderate weight; may work in dangerous, high crime areas of the community; and/or standby, callback and work during non-routine hours.

    Knowledge, Skills, and Abilities

    Thorough knowledge of: modern principles, practices and methods of public health nursing including public health programs, community, clinic and special programs, and school nursing programs.

    Considerable knowledge of:
    federal, state and local laws and regulations governing public health; the causes, control and care of chronic and communicable diseases and illnesses, handicapping conditions, mental illness and other disabling conditions; child growth and development and the principles and procedures involved in promoting maternal and child health and in family planning, pre- and post-natal care, and sexually transmitted disease control; and environmental, sociological and psychological problems related to public health nursing programs.

    Working knowledge of: principles and methods of effective supervision and training; community resources available for assistance and program outreach in public health programs.

    Ability to: Plan, organize and supervise the work of public health nurses and other personnel in a clinic setting or assigned program area; develop and maintain effective public relations with patients, staff, other County departments, community groups and organizations; recognize social and community problems which affect health of individuals, families and the general population; exercise independent judgement, analyze and evaluate situations and take appropriate action; instruct and prepare specialized educational programs; write effectively including the development of program goals and procedures for implementation; adapt public health policies and programs to the problems of an assigned area; and prepare clear and concise records and reports.
